First, mitigate this from ever happening. Tell someone where you're going and when you'll be back. Best person to tell is someone you see everyday. Study maps on the area you'll be in, bring the proper gear and prepare yourself for what you'll encounter.
Second, don't forget your backpack with your kit. Or your cell phone.
Third, stop. Sit down and relax. Take some time to gather your bearings and think about where you came from and how you got where you are at. If you really are lost the best thing to do is wait. If you decide to leave, mark your location and the direction you're traveling.
If you're on a river, you should be able to follow it upstream or downstream - back to where you came from.
Most importantly - before you hit the river or any wild place, you need to know some survival skills. You'll need shelter and fire (unless you're in the desert). Then you'll need water. Then you'll need food. You will also need to know how to navigate on land and make improvised signals.
Get a book from the library and learn some stuff before you leave. You can take it one step further and make a waterproof notebook of some crucial skills and put it in your fly fishing backpack (that's what I did).
